Have you ever had an X-ray for joint pain, only to be told that everything looks normal? If you’re still experiencing pain, stiffness, clicking, swelling, or difficulty moving the joint, you may wonder how that is possible.
The good news is that a normal X-ray doesn’t necessarily mean that nothing is wrong. X-rays are an important tool for diagnosing orthopedic conditions, but they primarily show bones and certain changes in the structure of a joint. Many sources of joint pain involve soft tissues or other structures that aren’t easily visible on a standard X-ray.
Understanding what an X-ray can and can’t show can help explain why you may still have pain even when your imaging appears normal.
What Does an X-Ray Show?
X-rays use a small amount of radiation to create images of the body’s internal structures. They’re particularly useful for evaluating bones and the overall alignment of a joint.
An X-ray can help your provider identify conditions such as:
- Broken or fractured bones
- Joint alignment problems
- Certain types of arthritis
- Bone spurs
- Significant loss of cartilage space
- Other changes affecting the bone
However, an X-ray doesn’t provide a complete picture of everything happening inside or around a joint.
Structures such as many tendons, ligaments, muscles, cartilage, and other soft tissues are not shown in the same detail on a standard X-ray.
That’s why a normal X-ray doesn’t always mean a pain-free joint.
What Can Cause Joint Pain When an X-Ray Is Normal?
There are several possible explanations for ongoing joint pain despite a normal X-ray. The cause depends on the joint involved, your symptoms, your medical history, and whether you’ve experienced an injury.
1. Tendon or Ligament Injuries
Tendons connect muscles to bones, while ligaments connect bones to other bones and help stabilize your joints.
Injuries to these structures may not be visible on a standard X-ray. For example, shoulder pain could be related to a rotator cuff injury, while knee pain could involve a ligament or meniscus problem.
An MRI or other imaging study may provide a better look at these soft tissues when your provider suspects an injury.
2. Cartilage Damage
Cartilage helps cushion and protect the surfaces of your joints. Cartilage damage can cause pain, stiffness, swelling, clicking, or a catching sensation.
Some cartilage problems may not be obvious on a standard X-ray, particularly in the early stages. Depending on your symptoms and examination, your provider may recommend additional imaging to better evaluate the joint.
3. Early Joint Changes
Some conditions can cause symptoms before significant structural changes become visible on an X-ray.
For example, arthritis can develop gradually. While X-rays can show certain changes associated with arthritis, the amount of pain a person experiences doesn’t always perfectly match what appears on an image.
Two people can have similar X-ray findings but experience very different levels of pain.
4. Muscle or Tendon Problems
Muscle strains, tendon irritation, and other soft-tissue conditions can cause significant pain without producing an obvious abnormality on an X-ray.
This is one reason your provider will consider more than just your imaging results when determining what’s causing your symptoms.
5. Joint Instability
If a joint feels like it is giving way, shifting, or moving in a way it shouldn’t, an underlying ligament or other soft-tissue problem could be contributing to the instability.
Your provider may use a physical examination along with imaging to determine what is causing the problem.
6. Referred Pain
Sometimes the source of pain isn’t actually the joint where you feel it.
For example, problems involving the spine or nearby nerves can cause pain that travels into an arm or leg. This type of referred pain may not show up on an X-ray of the painful joint because the source of the symptoms is somewhere else.
Why Your Physical Exam Still Matters
Imaging is only one part of diagnosing an orthopedic condition.
Your provider will also consider:
- Where your pain is located
- When the pain started
- Whether an injury occurred
- What activities make your symptoms worse
- Whether you have swelling or stiffness
- Your range of motion
- Joint strength and stability
- Your medical history
- Previous treatments and their results
Your symptoms and physical examination can help your provider determine which structures may be contributing to your pain and whether additional testing is necessary.
In some cases, your provider may be able to diagnose and treat the problem without additional imaging.
What Happens If My X-Ray Is Normal but I Still Have Pain?
If your pain continues despite a normal X-ray, your provider may recommend additional evaluation.
Depending on your symptoms, this could include:
MRI
An MRI uses magnetic fields and radio waves to create detailed images of soft tissues and other structures. MRI can be useful when evaluating conditions involving tendons, ligaments, cartilage, muscles, and joints.
For example, MRI may help identify a meniscus or ligament injury in the knee or a rotator cuff or labral problem in the shoulder.
CT Scan
CT scans use multiple X-ray images to create detailed cross-sectional images of the body. They can provide more detail about bones and complex joint structures than a standard X-ray.
Ultrasound
Ultrasound uses sound waves to produce images of certain soft tissues. Depending on the area being evaluated, it can help providers assess tendons, muscles, and other structures.
Arthrogram
In certain situations, an arthrogram may be recommended. During an arthrogram, contrast is placed into a joint before imaging is performed. This can help provide a more detailed view of structures such as cartilage, the labrum, ligaments, and the joint capsule.
The appropriate imaging test depends on your symptoms and what your provider is looking for.
Do You Always Need an MRI After a Normal X-Ray?
No.
A normal X-ray doesn’t automatically mean you need an MRI.
Your provider will consider your symptoms, physical examination, medical history, and the type and duration of your pain before deciding whether additional imaging is appropriate.
Sometimes conservative treatment—such as physical therapy, activity modification, medication, or other non-surgical treatments—may be recommended first.
In other situations, additional imaging may be helpful in determining the cause of persistent symptoms.
The goal isn’t simply to order more tests. It’s to use the right diagnostic tools to understand what’s causing your pain and determine the most appropriate treatment.
When Should You See an Orthopedic Specialist?
Consider scheduling an orthopedic evaluation if your joint pain:
- Continues for several weeks
- Keeps coming back
- Gets worse with activity
- Causes swelling or stiffness
- Limits your range of motion
- Causes clicking, catching, or locking
- Makes the joint feel unstable
- Interferes with work, exercise, sleep, or everyday activities
- Continues despite rest or other conservative treatments
You don’t have to wait until your pain becomes severe to seek care. An orthopedic specialist can evaluate your symptoms, review previous imaging, and determine whether additional testing or treatment may be appropriate.
A Normal X-Ray Doesn’t Mean Your Pain Isn’t Real
If you’ve been told your X-ray looks normal, but you’re still experiencing pain, you’re not imagining your symptoms.
X-rays are an important part of orthopedic diagnosis, but they are only one piece of the puzzle. Some conditions affect structures that aren’t clearly visible on a standard X-ray, while others may require a physical examination or a different type of imaging to identify.
